What is the Lachman Test?
The Lachman test is a fundamental clinical examination maneuver used by orthopaedic surgeons to evaluate the structural integrity of the anterior cruciate ligament (ACL). The ACL is one of the primary stabilizing ligaments in the knee, responsible for preventing excessive forward movement (anterior translation) of the tibia (shin bone) relative to the femur (thigh bone), as well as controlling rotational loads.
Because of its high sensitivity and reproducibility, the Lachman test is widely considered the expert gold standard for detecting ACL ruptures in a clinical setting, particularly in the acute phase of injury.
Why is the Lachman Test Performed?
An orthopaedic surgeon will perform this test when evaluating a patient for:
- Acute sports injuries, especially those involving sudden stops, pivoting, or twisting mechanisms (e.g., football, basketball, skiing).
- A reported “pop” sound or sensation at the time of injury.
- Rapid onset of hemarthrosis (joint swelling filled with blood) within hours of a trauma.
- A history of the knee “giving way” or feeling unstable during routine activities.
- Clinical correlation prior to confirming the diagnosis with an MRI scan.
How is the Lachman Test Performed?
Proper technique is critical for an accurate assessment. The examiner must ensure the patient is relaxed to prevent muscle guarding.
- Positioning: The patient lies supine (flat on their back) on the examination table. The examiner slightly bends the affected knee to approximately 20 to 30 degrees of flexion. This specific angle is crucial because it unlocks the knee and relaxes secondary stabilizers like the iliotibial band and posterior joint capsule, isolating the ACL.
- Hand Placement: The examiner uses one hand to firmly grasp and stabilize the distal femur (just above the kneecap). The other hand grasps the proximal tibia (just below the joint line).
- Execution: While holding the femur steady, the examiner applies a brisk, anteriorly directed pulling force to the tibia, assessing how far the bone moves forward and the quality of the stopping point.
Interpretation: Grading and Endpoint Quality
A normal, healthy ACL allows for a small amount of movement (2–4 mm) but stops abruptly with a firm, hard endpoint. When the ACL is torn, the tibia shifts further forward than the uninjured opposite knee. This abnormal translation is graded as follows:
- Grade I: 1 to 5 mm of increased anterior translation.
- Grade II: 6 to 10 mm of increased anterior translation.
- Grade III: Greater than 10 mm of increased anterior translation.
The Endpoint: Experienced orthopaedic surgeons often rely more on the “feel” of the endpoint than the exact millimeter measurement. A “soft” or “mushy” endpoint—where the tibia feels like it glides forward without a definitive sudden stop—is highly indicative of a complete ACL tear.
Common Clinical Pitfalls
Certain factors can complicate the examination, which is why expert evaluation is necessary:
- Hamstring Guarding: If the patient is in severe pain or anxious, they may involuntarily contract their hamstring muscles. This pulls the tibia backward and can result in a false-negative Lachman test.
- The “Pseudo-Lachman” (PCL Injury): If the posterior cruciate ligament (PCL) is torn, the tibia sags backward at rest. When the examiner pulls it forward to the normal neutral position, it may feel like increased anterior translation, mimicking a torn ACL. Checking for a posterior sag sign first prevents this error.
- Severe Swelling: A massively swollen knee may be difficult to bend into the required 20-30 degrees of flexion.
Orthopaedic Pearl: The Lachman test is significantly more sensitive than the traditional Anterior Drawer test (which requires 90 degrees of flexion). At 20-30 degrees, the joint capsule is relaxed, hamstring guarding is minimized, and a swollen joint is more easily positioned.
Accuracy Compared to Other Tests
- Highest Sensitivity: The Lachman test has a sensitivity of approximately 85-95% for acute ACL tears, making it the most reliable initial physical test.
- Versus Pivot Shift: While the pivot shift test is highly specific for confirming rotatory instability, it is often difficult to perform on an awake, guarding patient in the clinic. The Lachman test is much better tolerated acutely.
